64M with HTN/T2DM in septic shock from pneumonia. After 3 L crystalloid, on norepinephrine 0.18 µg/kg/min and VC ventilation (VT 6 mL/kg PBW, PEEP 8, FiO₂ 50%). At 4 h: MAP 68, HR 118, CVP 10; lactate 3.8→5.1 mmol/L, UOP 10 mL/h, Cr 1.0→1.8 mg/dL. Echo: hyperdynamic LV, no RV dysfunction. PLR increases LVOT VTI 16%; PPV 14%.
